How to Submit Parental/Guardian Consent Form

This guide is to assist users in submitting parental/guardian consent for underage users wishing to participate in LVUP.GG arenas.

Please carefully read and follow the steps below. If every information is correct and in order, the consent will be accepted within three (3) business days and the user will be allowed to participate in the arena.

The submitted documents will be maintained by Bigpicture Interactive for one (1) year, and shall immediately be removed/deleted upon the request of the participant or his/her parent/guardian. (However, please note that after deletion/removal of the documents the participants shall have to submit the consent again in order to participate in the arena)

Parental/Guardian Consent Guide for Team Leaders

1. Enter arena page, click the Registration button, and click the checkbox to agree with Arena Rules.
   
   
   
2. When the Parental/Guardian Consent window pops up, click the Download button to download the consent form.
   
   
   
3. Print out the LVUP.GG Parental/Guardian Consent Form.
   
   
4. Read the Consent Form carefully and fill out the required information in handwriting.
   
   
5. Participants and his/her parents/guardian should sign the form in the respective sections.
   
   
6. After completing the form, scan or take a photo of the Consent Form, enter the Arena page and attach the image on the Consent Popup to submit the form.
   
   
   
7. Once the form is submitted and is under review, the ‘Review’ modal should say, ‘Parental Consent for Underage User Under Review.
   
   
   
8. The confirmation process may take up to three (3) business days. Please note that after confirmation the user will be allowed to participate in the arena.
